How to Enroll in Provider Partners Health Plans

  1. Talk with a Licensed Agent Review your options, confirm eligibility, and understand plan details with a licensed and appointed agent.
  2. Complete Your Enrollment Submit an enrollment form and receive your welcome materials, including your plan ID card and coverage information.
  3. Get the Care You Deserve Access ongoing care, trusted providers, and ongoing member support.
